1st field value controls display of 2nd field value

I am trying to create a form where the value of 1 field limits the value options of a 2nd field. For example: Say I have a database of inventory in 5 warehouses. I want to create a form that will allow me to first choose a specific warehouse and then choose a specific item that is in the warehouse. The resulting value that is recorded is that 1 item from that 1 warehouse.

How do I create this type of relationship?
