Displaying query results in a form based on selected criteria

Hi
I have created a form which is derived form a query the query has a
prompt box which prompts the user to input the criteria they wish to
see.
I have worked out how to put a command button into the form and when
they click this button the prompt box appears. but the format changes
completely in that it returns it in a simple query format and not in
the form format that I wish to see it in.
HOw do I get access to run or even open the form again to give the
prompt box without having to close and open the form each time, and to
provide it in the format required...
does this make sense..

any help would be much appreciated...
