Image disappears during extract from Access to Word

Any suggestions? The property values are all correct... it disappears. It never used to, but we had someone try and alter the page by making the icon smaller to have less space between the icon header and the body of the document, and now whenever we hit export it disappears.

Any suggestions are welcome!
