How does one get a form to default to a new/blank record upon opening?

In design view, on the Properties dialog, set Data Entry to "Yes".

If you wish to use the same form for editing, there are several approaches,
but the simplest is likely to set Data Entry to Yes, but provide a
CommandButton with code in its Click event to change the related settings.
