Dependant boxes choose record or create new record

I have a Table with vacation information. On my form I have two combo boxes dependent in the following order. First box chooses CaseWorker and second box chooses Consumers belonging to that case worker. What I want to happen is if there is already a record that meets that criteria then it will display for editing, and if not a new record will be created. I have tried several different scenarios with subforms and single forms and cannot figure this out. Any guidance would be greatly appreciated.

You need to create a form which is bound to the dataset (table or updatable query).
Add the two selection comboboxes at the top.
In the AfterUpdate event change the Filter property of the form.
It will select the record found or have an empty form if not.
