Using Docmd.ApplyFilter

I need help implementing a form (Form2) filter using the following criteria:

a. 5 check boxes referring to the 5 Mailing List fields in 'Representative' table.
b. 'Event Attd/ Svc Used' combo box from 'Recruiting Activity' table.
c. 'Date of use' textbox from 'Recruiting Activity' table.
d. 'Date of addition' textbox from 'Employer' table.

Output:
a. Datasheet form.
b. Word document.

The filter will be based upon data from various tables, and generate a table in the form of a query table within the subform.
