losing data

When both my co-worker and myself enter data into forms (not the same
form) on our data base, sometimes some of the data is lost. Can anyone
tell me why this is and what can be done to stop this problem.
P.S. There is no rhyme or reason as to which data is lost.

Undo'ing the data. Not saving the data. Pressing the ESC key a couple
of times. Canceling the update.
