Partially clear form inputs

I have a form which opens up at startup. There are 4 fields which the
user inputs, which feed into parameter queries that are automated by a
macro. After this macro is done running, I would like the form to
reappear with only the last field still filled in.

In other words, I want the first 3 fields to be cleared out
automatically so that the user can use this tool multiple times without
having to re-enter that last field. I can't figure out how to make this
happen.
