I do not know of any templates, or sample, databases that fit your
description. And with terms as un-precise as "issues" and "images", I am not
at all sure what you want, in detail.
FYI, there is no business model that makes it worthwhile for anyone other
than Microsoft to develop templates. They do so to promote use of the
product. Those of us who create samples and examples for free normally
create entire databases and publish them "open source" so they can be
modified, instead of going to a lot of extra work to put the modifications
behind a set of forms to hide them from the user.

Basically the database keeps track of system images and system issues.
1 Issue can relate to many different images. Issues are always the
product of the Image, so I want to create 1 form that will show image
data, but allow the user to create data for this image from another
table called issues.
While this may seems complex in description, I assume/theorize that
this sort of database is created all the time. I was wondering if
anyone has published templates like this?
