OT. Fax Server With Access

What is a good Fax Server where I could present a form where a person
selects a person, presses a button, and I fax a cover page (which may be
an email body) and one or several attachments to the person selected?
The key is that it should be able to handle several documents and either
fax PDFs, Word docs, rtfs, txt, TIFF, and most of the types of documents
that one could send.

Any comments?
