A report I have can show new objects by looking in the system tables. Is
there a system table that keeps track of design changes and by whom? Or
maybe the On_Close event can append a record from the design mode?

Some would say, prevent your users altering anything!
Nothing built-in apart from the date of the most recent change to each
object. However this could be due to something trivial like sorting
A-Z or even compacting the database in some cases.
If you program this yourself you could capture when and who changed
things but how about why?
I expect someone has done something along these lines - I looked in
Google but as usual the problem is sorting out which items refer to
Access (the database), since so many articles contain the word access
in the normal sense. The usual tricks (eg adding "vba") didn't help.
