Calculate wih a Combo Box

I need to create a form call "Receipt" that contain field to calculate
payment - called " TotalPayment" = "PaymentFor" * "Period". In
"PaymentFor", I suppose to use a combo box for 5 items selected, but
only 2 items have fixed rate; another 3 items doesn't have.

1. I create a combo box for "PaymentFor" is correct or not?
2. In the field "PaymentFor" will have 5 items, but only 2 items have
fixed rate, and it should be adjustable from time to time. How to
create this combo box?
3. In the field "TotalPayment" = "PaymentFor" * "Period". If I select
the item in "PaymentFor" without fixed rate, I hope I can input
"TotalPayment" directly.
