Simple html forms out of a database record?

2 things:
1) there any simple (and I mean REALLY simple cookie cutter) way to
export a TEXT format or HTML format document or report out of a single
ACCESS record?

-Just take a record and export the fields into a document the way you
can use handbase on a palm and make reports out of it?

-It seems like it should be a glaringly easy thing to do but there
doesn't seem to be anywhere where it's documented how to do this in an
easy way.

-I want to just be able to make a document that's something like this:

This guy went to <field1> park and bought some <field2> <br>. He's a
very <field4> <field3>

2) Just want to be able to create a simple FORM in WORD (NOT in the
stupid ACCESS FORM) to take in the info into a record in ACCESS.

It doesn't seem like these things should be hard, but for some reason
it seems to be far from simple. I know there's about a billion people
who would like to do things this way as well.

Perhaps I'm just slow or don't read documentation well enough.

Any help much appreciated.
Thanks in advance

Hi
Not the most tactful way to ask, how about trying a word group?
