Regarding Reports And Queries!

I have created a Query in MS Access which gives output comparing
month, year and name of the employees. Now i have connected this query
to a report and the report is coming out fine too. But when i trie to
take these inputs from a form and call the report using a macro, it is
giving a blank report. I dont know where i went wrong. The name i gave
for comparing in the query is also same and everything seems correct.
Can you plese help me out with this.

Show the SQL view of your query, describe in detail your data structure,
show the macro, and describe in detail the form. Then, perhaps someone can
offer worthwhile suggestions.
