Are you trying to create the new form in code?
Or in the Design environment?

What version of Access are you using?

Can you create a new form in a new database?
Or does this problem occur only when you try to add a form to one particular
existing database?

You probably need to register the DAO dll using regsvr32. For example regsvr32
"C:\Program Files\Common Files\Microsoft Shared\DAO\DAO360.DLL"
