Populating Form fields with data from a seperate table

I am creatig a database in 2000 where I need to be able to have the
users add data and information into lookup tables. That is no
problem, I have a form for that, that works fine. This information
will then be used for a combo box on a form that will pull the
"queried information" based on the selection. What I need to do, if
it is possible is to take those values that are visible in the combo
box (from that seperate table) and have the information that is
related to them, show up on a form as individual fields so the users
can enter information that is directly related to them on a montly
basis.

Baically, I am having people track traffic on servers that have a
number of different websties on them. There may be additional sites
that get added, or renamed, deleted, etc. I don't want to have to
change the form, table, report, etc with every addition or change.
When the user selects the server from the combo box, I want for the
list of sites some have 1 some have 20, but I want tehm all to be
listed as individual records in a field on the form/subform so the
statistical data collected can be entered (#of hits) on a montly
basis.

HELP please... and if my logic on this plan is totally in th wrong
direction, I am open to ideas to change that as well.
