Table fileds Vs Records

Can any one helpme?

I'm a new user of ms access 2000 and need to do a db where checklists will
be filled.
My problem is to create a system where de records of table 1 are de fileds
of table 2.
The main issue is to simply drop a certain number of questions in table1 and
assume that de answer to each question is registered and linked in table 2,
each visit.
