error on form

I have a form in my database that has a field that looks to a table,
and I have set it so if not in list, it will automatically add to the
table. It worked fine when I created it, but after I closed the
database and reopened it, and I try to enter data in the field,
instead of asking if I want to add the data, I get an error -
connection to the network has been lost. I created the database on my
hard drive, not a netowrk drive.

Any suggestions on why I am getting this error or how I can fix it?
