(Web form & data retrieval)

I am looking to build a form whereby I can input a customers information (Name, address etc). However, what I want to do is for the to check a box then upon submission data will be retrieved relevant to the checked box.

So for exampleL the user is inputing their name etc and choose the type of car they have. The information regarding the car is then automatically put into the form for me to look over.

I know how to produce a form through Zoho etc, but what I am struggling with in finding something that will retrieve data.
